Month four is where recovery is actually decided, and almost nothing is built for it.
Men finish the eight-week Iron Ridge IOP in good shape. They have interrupted the pattern, they understand what was driving it, they have done real work, and they leave with a written plan. Then the structure disappears.
That is the moment aftercare coaching exists for.
Why the eight weeks are the easy part
Structure holds almost anyone. Nine to twelve hours a week, a group that knows your whole history, a licensed provider you see every week, and a schedule that accounts for the hours that carry risk. Very few men struggle inside that container.
Then it ends. The crisis that brought you in recedes. Your partner stops asking every day. Work gets busy again. The week goes ordinary — and ordinary is the test the program was never able to give you.
The men who hold onto the change are, with very few exceptions, the ones who were still in something at month six. That is the single clearest pattern in this work, and it is the whole argument for aftercare.
What aftercare coaching does
Keeps the cadence. Two sessions a week plus reasonable daily contact. Considerably lighter than 9 to 12 hours, considerably heavier than nothing. See structure and cadence.
Keeps the plan alive. You left the IOP with a written continuing-care plan — early warning signs, high-risk contexts, a lapse protocol. A plan nobody reviews becomes a document in a drawer within about six weeks. Aftercare coaching is where it gets revisited and revised as your life changes.
Watches the drift, not the behavior. The useful work is nowhere near the behavior. It is in the small renegotiation weeks earlier — the reason that sounds entirely reasonable, the meeting swapped for a virtual one, the check-in pushed to next week. Those are easy to notice from outside and nearly invisible from inside.
Handles the transitions. The first business trip. The first serious argument after things had been going well. The first stretch alone in the house. The first time something goes genuinely well and the reward instinct shows up. Each of these gets planned for in a calm state.
Absorbs the accountability role. So your partner does not have to. Rebuilding a relationship while also functioning as the monitor is not a realistic ask of anyone, and it is one of the most reliable ways for a recovering marriage to stall.
What it does not replace
Aftercare coaching is a non-clinical service. It does not diagnose or treat any mental health condition and is not a substitute for therapy or medical care.
For most men leaving the IOP, continuing with a licensed provider is the load-bearing piece of continuing care, and coaching runs alongside it rather than instead of it. Where your continuing-care plan calls for ongoing licensed care, coaching is not a way to skip it — see coaching vs. therapy.

If you lapse
Say it within twenty-four hours.
That is the entire protocol and it is the most consequential habit in the whole arrangement. A lapse disclosed quickly is a workable event — you look at the sequence, find where the drift started, and revise the plan. A lapse that goes underground takes weeks of secrecy with it, and the secrecy does far more damage than the lapse did.
The men who do badly after the program are rarely the ones who lapsed. They are the ones who lapsed and disappeared, because they concluded that having failed they had forfeited the right to ask for help. That belief is wrong and it is worth deciding it is wrong now, before it gets tested.
For alumni of other programs
You do not have to have completed the Iron Ridge IOP. Men finishing residential or intensive programs elsewhere face exactly the same month-four problem, and the discovery call works the same way.
What it costs
$4,500 per month, month to month. Stop when the structure is no longer what you need, and come back if it is again.
